Prior to your appointment, medical history and consent forms will be sent to the email address on file. Please fill these out before your appointment, or plan to arrive 15 minutes before your first scheduled appointment in order to complete the forms. You will also meet with one of our specialists so that we can get to know you better, take a look around the clinic, and learn how we can help you on your wellness journey!
You are able to schedule appointments through our Covery App or online through the booking portal. We are also available to schedule you over the phone, via email, or in person.

We do not accept insurance at this time. However, we are partnered with Cherry Financial who can provide payment plan assistance. This will be a soft credit check and will not affect your credit score. FSA/HSA may be used on most medical services.
